Content Type of Documents set as folder on in SharePoint Library

I have a document library with both version control and content approval enabled. The library uses one custom content type for documents and is configured to include the "New Folder" button on the New menu.

I noticed an issue where the columns specified in the associated content type were only displaying for documents that had been in the library for a while. Newer documents only gave me the option to change the title. I modified a view of the list so that the Content Type was displayed for each document, and found that the majority were set as the "Folder" content type even thought they were documents. I was able to resolve the issue by adding a second content type to the library, the re-selecting the correct one for each document.

Once there were two document content types associated with the library, the Content Type field was displayed on the form when editing the properties of a document. When I first open the "Edit Properties" page for a document, it has the content type that I wish to use selected, but the description underneath says "Create a new folder". If I try to save the properties, a page with an error was displayed. I needed to select the second document Content Type added to the library temporarily, then reselect the original again for it to register properly and for the Content Type metadata to save.

Once the correct content type was selected, the fields associated with the document content type were available again when modifying the properties of a document in the library.
